Prescribing intravenous fluids

A structured guide to assessing a patient and prescribing intravenous fluids safely.

1. Introduction

Safe intravenous fluid prescribing requires assessment of indication, volume status, electrolyte needs and relevant comorbidities. Fluids should be prescribed for resuscitation, routine maintenance, replacement or redistribution based on the clinical picture.

2. Assessment

Review the indication, current observations, fluid balance, recent blood tests, urine output and relevant history including heart failure, renal disease, liver disease and sepsis. Examine the patient for signs of hypovolaemia or fluid overload.

3. Prescription

Choose the fluid type and rate appropriate to the indication and the patient''s comorbidities. Reassess frequently, monitor electrolytes and avoid routine use without a clear indication.

4. Completion

Document the indication, monitoring plan and review point clearly. Escalate if the patient is unstable or fluid requirements are complex.
