1. Introduction
Urethral catheterisation is a core clinical procedure used for bladder drainage, monitoring urine output and selected perioperative indications. It should only be performed when clinically indicated and using aseptic technique.
2. Preparation
Confirm the indication, check for contraindications such as suspected urethral injury where relevant, identify the patient correctly, explain the procedure and gain consent. Ensure privacy and a chaperone when appropriate. Assemble equipment and perform hand hygiene.
3. Procedure
Position the patient appropriately and clean the urethral area using aseptic non-touch technique. Prepare the sterile field, instil anaesthetic lubricating gel where appropriate and insert the catheter gently until urine flows. Advance slightly further before inflating the balloon with sterile water according to the manufacturer''s guidance, then withdraw gently until resistance is felt. Attach the drainage system and secure the catheter.
4. Aftercare
Ensure free drainage, position the bag below bladder level and maintain dignity throughout. Dispose of equipment safely, document the procedure including catheter type and balloon volume, and monitor for pain, bleeding or failure to drain.
